Super Choice - Fund Nomination Form
swimEC has developed two forms that a fund can use to help its members nominate the fund as a chosen fund:

• The Super Choice - Fund Nomination Form provides all the information the member needs to provide to their employer about their chosen fund.
  
  
• The 'Choice of Superannuation Fund' letter from the trustee provides the member with the required 'complying fund' and 'ability to accept contributions' statements.

The forms are accompanied by Guidelines for the preparation and customisation of the documents.
